Voided Check – If you would like refunds direct deposited
Form 1095-A – If you obtained health insurance through healthcare.gov
Amounts paid and dates of all Federal Estimated Payments (if applicable)
Amounts paid and date of Federal Extension Payment (if applicable)
Amounts paid and dates of all State(s) Estimated Payments (if applicable)
